Client is a leading nonprofit organization in New York City, dedicated to transforming lives through innovative behavioral health, primary care, and social services. Our network of over 60 community-based agencies serves more than 150,000 New Yorkers annually, focusing on reducing hospital readmissions and improving care transitions for individuals with serious mental illness, chronic health conditions, and substance use disorders.

Position Overview

The Director, Hospital Partnerships and Care Transitions is a pivotal leadership role responsible for fostering and maintaining collaborative relationships with hospital systems to enhance discharge planning, care transitions, and community-based care. This position oversees the centralized intake unit, provides clinical guidance, and drives data-informed strategies to improve patient outcomes and operational efficiency.

Key Responsibilities

  • Partnership Development : Build and maintain strong relationships with hospital systems to streamline discharge planning and ensure seamless transitions to community-based care.
  • Centralized Intake Oversight : Manage and supervise the centralized intake unit to optimize patient referral processing, assessments, and service coordination, minimizing wait times.
  • Clinical Guidance : Provide clinical oversight to intake staff and support agency-based care teams in managing complex cases and ensuring high-quality care delivery.
  • Data-Driven Performance : Utilize data to monitor key performance indicators (KPIs) such as hospital readmission rates, discharge success, and patient satisfaction. Implement performance improvement strategies based on data insights.
  • Crisis Response : Participate in on-call crisis response as needed to address urgent patient needs.
  • Collaboration : Work with team leaders to align intake processes with organizational goals, ensuring efficient patient flow and resource allocation.

Qualifications

  • Experience : Minimum of 5+ years in hospital care coordination, discharge planning, or community-based care systems.
  • Leadership Skills : Proven ability to lead and supervise intake staff and field-based care teams, with strong interpersonal and communication skills.
  • Data Proficiency : Experience using healthcare data and performance metrics to drive clinical and operational improvements.
  • Education : Advanced degree in healthcare, social work, public health, or a related field preferred.
  • Language Skills : Fluency in English required; bilingual proficiency (e.g., Spanish) is a plus.
